  • COVID-19 testing limited to FFCRA / CDC guidelines or medical necessity, outpatient settings only

  • Preventive breast cancer mammography screening — limit 1 per plan year
  • Routine preventive/wellness visits (men, women, children) — limit 1 per plan year
